NEED HELP APPLYING FOR HEALTH INSURANCE?

Michigan’s Health Insurance Marketplace is open for uninsured people to find and purchase health coverage, and for those who were covered in 2014 to reapply for next year.

You’re considered covered if you have Medicare, Medicaid, CHIP, any job-based plan, any plan you bought yourself, COBRA, retiree coverage, TRICARE, VA health coverage, or some other kinds of health coverage. Plans purchased between now and Dec. 15 will go into effect on Jan. 1, 2015.

Open enrollment for 2015 health insurance coverage closes on Feb. 15, 2015. Be sure to visit the Marketplace and enroll in a plan before this date.

Get Health Insurance Application Assistance from WSUPG

The Centers for Medicare & Medicaid Services (CMS) has designated Wayne State University Physician Group as a Certified Application Counselor to help guide people through the insurance enrollment process.

Our financial counselors have gone through special training to become certified to help you understand your health coverage options and enroll in a plan.

Appointments are required to speak with a WSUPG insurance application counselor. Please do not go to a WSUPG location without first scheduling an appointment.

What to Bring to Your Insurance Enrollment Appointment

Once you have scheduled an appointment with our financial counselors, please be sure to bring with you:

  • Basic contact information
  • Names of individual(s) seeking health coverage
  • Personal information for each applicant (e.g., name, date of birth, relationship to the person filing the application, social security number)
  • Family and household structure (e.g., single parent, child custody, other parent’s name, date of birth)
  • Income information individual or household income information
  • Information regarding access to other health coverage
